The West Yorkshire and Harrogate Integrated Stroke Delivery Network (ISDN) aspires to establish a seamless, high-quality stroke care system across the region, ensuring that patients receive timely, effective, and evidence-based care from the moment a stroke occurs through to rehabilitation and long-term recovery.
By connecting services across primary, secondary, and tertiary care, as well as prevention, pre-hospital care, rehabilitation, and community support, the network seeks to improve patient outcomes and reduce disparities in stroke care.
This collaborative model aims to bring together a broad range of stakeholders, including partners from the Voluntary, Community, and Social Enterprise (VCSE) sector, to create a truly integrated, patient-centred approach. By working together across all stages of care - from prevention to recovery and longer-term support - the network hopes to provide a comprehensive, whole-pathway service that meets the individual needs of stroke patients and enhances their overall care experience.
In April 2024, the ISDN transitioned from the West Yorkshire Integrated Care Board (WYICB) to WYAAT, launching a repurposing exercise to reengage stakeholders and drive our collaboratively developed workstreams forward.
In March 2025, we launched a new ‘All Things Stroke’ website, to support local patients, their families and carers with resources and support relating to stroke. Designed in collaboration with local stroke survivors, the voluntary sector, and healthcare professionals, the website acts as a ‘one-stop-shop’ and online directory for stroke advice and information, as well as support groups which can be filtered via location to show results in a user’s local community.
